Who am i?

So the state department wants proof that I've been using my current name for at least 5 years. Though they want 3 public records that show my name, date, place of birth, and show exclusive use of my new name for at least 5 years.

The hard part is something with my birthplace.

So what I've got to send them. two canceled drivers licenses one under my old name (issued 1989), one under my current name (issued 1996). 2 1099s & 1 W2. The two 1099s are from the same company, with the same address, social security number, just the name differes. Also to show some consistancy I'm tossing in a current W2. And just for fun I've got a voter registration card from 1996 and a 1998 bank statement

So no birthplace but I do have consistant social security numbers and drivers license numbers.

I hope it's enough.

They want something that shows your birthplace, with your current name? That seems weird; the only official document that I can think of that shows my birthplace is my birth certificate, which of course would have your original name. Do they give any examples of typical documents that one might have after a name change that show a birthplace and the new name?

The examples they give include, school records, military records, employment records, tax records, financial records, insurance records. I think on some medical and some insurance records might include ones birthplace. Though I think the forms with that information end up staying at your doctors as they're the forms filled out when applying for insurance or first seeing ones doctor.

Hi,

nice to meet you.

I already went into the post office, and submitted my paperwork. I'm currently dealing with a letter from the state department saying "We need more proof".

The instructions in the letter claimed that I should gather my proof and mail it back to them. They did include two copies of the "I've known this person for X years" form, though I was hoping to ignore that as finding time to get it notorized may be difficult.

So did you take all the paperwork in the first time or did you get the letter asking for more deatils from them?

BTW, looking at your friends list, you wouldn't have gone to the pasadena vagina monologues would you?

Okay... thanks for the advice... It looks like I'll be needing to find a notary this weekend. Hmm... I looked at the notice and it only had the 900 number on it.

I had no idea...

At first thought that wouldn't help as I'd really like to get my mother to sign off and she lives many miles away. But thinking about it, I have a friend who works in pasadena who has known me since before. Also clare has known me for a long time (heck with her palm pilot fetish she probably knows when we met).

Hmm... interesting... so where would I find your office, and would you be available monday?

(I can't remember why) I had to go IN to attest that my brother was who he said he was. We look a LOT a like, we have the same last name, our licenses had the same address on it (we still lived at home). The gal who processed his application was dim. She had a hard time getting in thru her head we were siblings (I can't remember if we both had our birth certificates or what). "No, I *really* have known him ALL my life". ::sigh::
